Step 1 - Receive care

You receive care for a qualified medical service. With UnitedHealthcare-branded hospital insurance, you choose where you want to receive care with no network requirements. However, remember, if you have a major medical plan that uses a network, you may want to stay in-network to get the most out of that plan, too.

Step 2 - Submit claim

You complete a claim form and submit it with copies of the required information. Your member hub at uhcmemberhub.com will have claim forms and instructions on what information to include.

Step 3 - Receive benefit

You receive a check for the fixed amount specified by your plan for that service (within your calendar-year maximum). Or if you choose, benefits will be paid directly to a hospital or other provider if you make an assignment of benefits.
